The app goes beyond providing access to patient medication lists and delivers an end-to-end suite of tools for management including discharge planning, where PersonalRX connects to the health care agency or rehab facilities electronic health records (EHR) to minimize errors through care team management for an individual patient and ensure medication lists are consistent across all patient records.   

A survey from Capital Caring Health found that 90% of Americans over 50 expect to age in place. To support patients’ maintaining independence at home, caregivers often oversee drug intake to ensure patients take their medications on the prescribed days, at the right time intervals and in the right combination. PersonalRX simplifies the process to give caregivers back hours per week. PersonalRX works with doctors’ offices to manage prescription changes, copays and refills, and ensures that patients receive a full 30-day supply of medications. Additionally, dose packs are labeled and come with instructions, so patients know they’ll receive the right medications at the right time.
